Social workers help patients, residents, and their family members understand their illness or condition and provide them with information about the resources available to them to cope with the emotional, financial, and social needs that arise with a diagnoses.
At Kittson Healthcare our social workers primarily assist with:
- Transition and discharge planning
- Connecting patients and family's to appropriate community resources and support services
- Social, financial, or emotional resources and support
- Providing comfort
- Help to understand patient rights and options
- Facilitating communication and family involvement
- Advocating for patient/resident rights
- Psychosocial assessments
